Please note carefully that the Declaration Forms signed by the Director SR in respect of Departmental employees as well as GDS employees alone should be used.

This time the Divisional Secretaries should submit 5 copies of List of Members along with the Declaration Forms to the Divisional Heads - Also note that the List and Declaration Forms are to be submitted only to Divisional Heads.
